Health History

Preparing for an In-Office Surgical Procedure

BEFORE YOUR VISIT:

  1. Do not eat, drink or smoke anything after midnight the night before your appointment- This includes all foods, all beverages, water, gum, candy and ice. Eating or drinking anything will make giving you anesthesia unsafe.
  2. Remove all facial piercings and jewelry that you normally wear and leave these items at home.
  3. Children and guests are not allowed in our office and waiting areas.
  4. You will be in the office about 3-5 hours on the day of your appointment, depending on any medical preparation needed before your procedure and recovery time after.
  5. You cannot drive to this appointment. It is unsafe and illegal to drive after having anesthesia. You can have a trusted person drop you off and return to pick you up or you can take a cab/ride-share home on your own.
  6. If you have a payment due, it can be made with cash, credit card, or money order. You can use more than one payment method if needed. If you are paying with a credit card, the card and the cardholder (with a picture ID) must be present in the office.
  7. If you develop a cough or any cold or flu symptoms before your visit, please call us at 312-707-8988.

Preparing for a Medication Abortion Pill

BEFORE YOUR VISIT:

  1. You can eat and drink normally.
  2. Children and guests are not allowed in our office and waiting areas.
  3. You will be in the office about 1-2 hours on the day of your appointment.
  4. You may drive to this appointment, have a trusted person drop you off and return to pick you up or you can take a cab/ride-share home on your own.
  5. If you have a payment due, it can be made with cash, credit card, or money order. You can use more than one payment method if needed. If you are paying with a credit card, the card and the cardholder (with a picture ID) must be present in the office.
  6. If you develop a cough or any cold or flu symptoms before your visit, please call us at 312-707-8988.

IUD Referrals

FPA has partnered with My-IUD.com to provide Mirena and Paragard IUDs for our patients, after an abortion procedure. If you are scheduled for an abortion and are interested in an IUD, please call us at 312-707-8988 and ask us to create a referral for you. My-IUD.com will contact you directly to get you preauthorized. My-IUD.com is unable to do authorizations on the same day as your visit. You must be preauthorized before your visit for this service. My-IUD.com has contracts with most private insurance companies, Illinois Medicaid, and Medicaid programs in many other states as well.

Clinic Information

The entrance for our main clinic is located at 659 West Washington Blvd, Chicago, IL 60661. We often have clinic escorts who wear bright pink vests. They can help you enter the office if protesters are present on the day of your appointment.
