Our goal is to provide essential reproductive healthcare for our patients during this challenging time, while minimizing risk of infection for our patients and staff. With this in mind, we ask that you:

Call us at 312-707-8988, if you have:

• Fever of 100.0 or more
• Cough
• Chills
• Sore throat
• Difficulty breathing
• Muscle pain that is new or different
• Headache that is new or different
• New loss of taste and/or smell
• Had recent testing for Covid-19
• Been exposed to the coronavirus (COVID-19)

Please tell our staff if you have any health conditions when you make your appointment online or over the phone. If you are flying in to Chicago from out of state or it will take you 3+ hours to drive to our office please let our staff know when you schedule your appointment.

We are only allowing patients in our offices at this time. Have your guest / support person drop you off for your appointment and come back to pick you up later, or have them wait in the car just outside the office. You may wait with them in the car when you are not being seen by our staff.

Unless you have a medical condition that makes it impossible, you must wear a face mask or cloth face covering while in our offices.

If your guest / support person has to sign paperwork or make a payment on your behalf they may come inside of the office briefly.

We thank you in advance for your patience at your visit, as we may have limited staff some days. Please do not make your appointment on a day when you have other plans or obligations.

If you need to cancel or reschedule your visit please let out staff know as soon as possible. You can call us or email us for your convenience. If you are uncomfortable providing personal information online, you can always call us.

When medically appropriate FPA will:

• Provide a phone consultations for patients who would normally return to the office for a follow-up visit

If you are ill, please do not not come into our office, we will make every effort to reschedule your visit for a date and time that is convenient for you. We can provide you a note for work or school prior to the visit stating that you have a scheduled appointment.

Please do not come into our office if you have been exposed to cononavirus (COVID-19) or if you have any symptoms of the virus including fever, cough, chills, sore throat, difficulty breathing, muscle pain that is new or different, headache that is new or different, or new loss of taste or smell.

• When you enter the office please use hand sanitizer and/or wash your hands
• After checking in and completing your initial paperwork, please let our staff know if you prefer to wait in your car and we will call you when it is your turn to be seen
• Only patients will be allowed in the office at this time unless your guest is briefly needed to make a payment on your behalf or to sign paperwork – you will still need to make arrangements for safe transportation home and may not drive yourself
• Please know that we are frequently cleaning all surfaces to help reduce risk, but we encourage you to wash your hands and/or use provided hand sanitizer throughout your visit
• We will be limiting patient interactions as much as possible, but please practice social distancing when/if you share space with other patients and our staff, staying at least 6 feet away from others whenever possible.
